What to Expect
Your Visit
You will be asked to complete a New Patient or New Problem Information from which outlines your medical history and our Notice of Privacy. Please download and complete this forms prior to your visit. In order to have the most complete and accurate information, bring a written list of any medications, including any over-the-counter medications and nutritional supplements you are currently taking. Also be prepared to list prior treatments and surgeries, even if they are not related to your current problem. If you need special assistance, please consider asking a friend or family member to accompany you. Let us know when you schedule your appointment if you have any special needs that we can assist with. Dress in loose clothing that is easy to take off and put on. You may be required to undress for the exam or x-ray procedures. You make bring any previous diagnostic studies that you may have completed prior to your visit (X-Rays or MRIs).

Download the Authorization for Release of Information and return the completed form to your local office.
Medication Refills
In order to complete prescription refills in a prompt manner, please call our office at least three days before your medication runs out. This will allow your surgeon time to review your prescription needs and complete your refill.
